What Is Pelvic Congestion Syndrome?

Pelvic Congestion Syndrome (PCS) is one of the possible causes of long-term pelvic pain. It occurs when veins in the lower abdomen and pelvis become enlarged and inefficient at moving blood. Much like varicose veins in the legs, these enlarged veins can cause aching, pressure, and heaviness.

While PCS most often affects women — especially those who have been pregnant — anyone with underlying venous insufficiency may experience it.

Common Symptoms of PCS

The most typical symptom is chronic, aching pain in the lower abdomen or pelvic region that lasts longer than a few months. Many people describe this pain as dull or pressure-like. Other common symptoms can include:

  • Deep ache in the pelvis or lower back that worsens with prolonged standing or sitting
  • Increased pain with activity, sexual intimacy, or during menstruation
  • Feeling of heaviness in the pelvis
  • Urinary urgency or frequency
  • Bloating or gastrointestinal discomfort

Keep in mind that you may have some or all of these symptoms. If you recognize these patterns, we encourage you to consult a healthcare provider for a full evaluation.

Getting Diagnosed

Pelvic pain can have many causes, so a thorough medical workup is important. This may involve a physical exam and diagnostic tests like ultrasounds, CT scans, MRIs, or specialized venography. Once a clear diagnosis is made, your doctor can help you explore the most effective options for relief.

Physical Therapy & Wellness Support for PCS

Although some people with PCS may require medical interventions such as vein embolization, physical therapy is a powerful, noninvasive part of care that can help manage your symptoms. Our team at Lokahi Physical Therapy & Wellness offers:

  • Gentle, hands-on techniques (manual therapy, visceral mobilization) to improve circulation and decrease pain
  • Targeted exercises to support the muscles of the pelvic floor and core
  • Techniques to reduce tension, improve posture, and promote relaxation
  • Personalized education to help you move comfortably and confidently in your daily activities

Our goal is to help you achieve long-term relief and enhanced well-being.
